Présentation du finastéride (Propecia)

Le finastéride est utilisé dans le traitement de l�hypertrophie de la prostate. Un des effets secondaires chez les patients traités à été la repousse des cheveux. Au cours d�études cliniques, le fabricant Merck a démontré que l�efficacité du produit contre l�alopécie était conservée à une dose cinq fois inférieure à celle utilisée pour le traitement de l�hypertrophie bénigne de la prostate, soit 1 mg/jour versus 5 ;mg/jour.

Le finastéride a été commercialisé sous la forme de comprimé à 1 mg en 1997 aux Etats-Unis et en 1999 en France (Propecia®).

Quel est le mécanisme d'action ?

Le finastéride est un inhibiteur de la 5-alpha-reductase, l�enzyme responsable de la réduction de la téstostérone en dihydrotestostérone, enzyme qui est elle-même responsable de la suractivité du follicule pileux.

Quelle est l'efficacité ?

Les essais cliniques sur des hommes de 18 à 41 ans démontrent une efficacité antichute après trois mois de traitement et une repousse pour les chanceux après sic mois traitement.

Après un an, il y a une repousse dans près d'un cas sur deux.

Après deux ans, il y a une stabilisation dans 80% des cas et une repousse dans deux cas sur trois.

Après quatre ans, il y a une légère perte, le temps faisant sont �uvre, mais 55% des patients gardent plus de cheveux qu'il n'en avait avant de commencer le traitement.

Les effets bénéfiques disparaissent 6 à 12 mois après l'arrêt du traitement.

Quels sont les principaux effets secondaires ?

Il y a très peu d�effets secondaires. Il s�agit principalement d�effet secondaires d�ordre sexuel pour 1 à 2 % des hommes traités (baisse de libido, troubles érectiles, anomalies de l�éjaculation). Ces effets disparaissent dans deux cas sur trois malgré la poursuite du traitement, et dans tous les cas deux semaines après l�arrêt du traitement.

Ont été aussi reportés beaucoup plus rarement : une sensibilité et un développement des seins, éruption cutanée ou �dème des lèvres.

L�encyclopédie en ligne Wikipédia dans sa version anglaise reporte de façon douteuse des effets secondaires pour 6 à 19% des patients, ainsi que la persistance des effets secondaires sur de longues périodes après la fin du traitement sans faire référence à des publications scientifiques.

Le finastéride n�est pas efficace pour la femme et ne doit pas être pris en raison de ses effets avérés sur le développement du f�tus.

Drug Uses

Propecia is a pill used for the treatment of male pattern hair loss on the vertex (top of head) and anterior mid-scalp area (middle front of head). There is not sufficient evidence that Propecia works for receding hairlines at the temples. Propecia is for use by MEN ONLY and should NOT be used by women or children.

How Taken

Propecia comes as a tablet. It is usually taken once a day and may be taken with or without food. Take Propecia exactly as directed. Do not take more or less of it or take it more often than prescribed by your doctor. Do not stop taking Propecia even if you feel well or without talking to your doctor.

Warnings/Precautions

Do not take this medication without talking to your doctor if you have liver disease. You may not be able to take Propecia, or you may need a lower dose or special monitoring during treatment. Propecia is intended for men use only. Women should not take it. Pregnant women should not take Propecia. Propecia is in the FDA pregnancy category X. This means that it is known to cause birth defects in an unborn baby. Women who are or who may become pregnant must not handle crushed or broken Propecia tablets. The medication could be absorbed through the skin. Propecia is known to cause birth defects in a developing male baby. Exposure to whole tablets should be avoided whenever possible however exposure to whole tablets is not expected to be harmful as long as the tablets are not swallowed. It is not known whether Propecia passes into breast milk. Do not take this medication if you are breast-feeding a baby.

Missed Dose

If you miss a dose of Propecia and you remember the same day, take it as soon as possible. If you do not remember until the next day, skip the missed dose and go back to your regular dosing schedule. Do not take 2 doses at once.

Possible Side Effects

Stop taking Propecia and seek emergency medical attention if you experience an allergic reaction (difficulty breathing; closing of the throat; swelling of the lips, tongue, or face; or hives). Other, less serious side effects have been reported but usually resolve with continued treatment. Continue to take Propecia and notify your doctor if you experience: -decreased libido (sex drive); -decreased volume of ejaculate; -impotence (trouble getting or keeping an erection); or -breast tenderness or enlargement. Side effects other than those listed here may also occur. Talk to your doctor about any side effect that seems unusual or that is especially bothersome.

Storage

Store Propecia at room temperature between 59 and 86 degrees F (15 and 30 degrees C) in a tightly closed container, away from moisture.

Overdose

Seek emergency medical attention if an overdose is suspected. Symptoms of a Propecia overdose are not known.

More Information

DO NOT share Propecia with others for whom it was not prescribed. DO NOT use Propecia for other health conditions. Keep this product out of the reach of children. If using Propecia for an extended period of time, obtain refills before your supply runs out.
